Daily Self-Certification Forms

Please make sure that you fill out the "Daily Required Health Questionnaire Self-Certification" form on a daily basis.  Your child must have one everyday before he/she goes to class.  Should you need more forms, call the front office and we will make sure that your child brings a packet of the forms home.
